Pay Range:
Compensation is based on the level and requirements of the role. Salary within our ranges may also be determined by your education and experience as required by the role, as well as internal equity and alignment with market data. Typically, new team members join at the minimum to mid salary range.

As an LPN in our Adult Residential Program, you will play a critical role in stabilizing, supporting, and restoring the health and dignity of individuals living with serious mental illness.

This role is designed for clinicians who want clear expectations, meaningful work, and the chance to make a real difference every day. You will be accountable for:
Direct Clinical Care
Provide in-person and phone-based triage for medical and psychiatric concerns within your scope of practice
Deliver basic health screenings and assist with chronic disease management
Administer medications and injections as prescribed by licensed providers
Monitor medication side effects and client wellness to ensure safe, effective care
Supportive, Whole-Person Interventions
Educate clients on nutrition, medication compliance, hygiene, and physical wellness as part of their mental health recovery
Deliver in-home skills training for chronic conditions like diabetes, Hepatitis C, and COPD
Collaborate with caregivers and support teams to help clients thrive in the least restrictive environment
Interdisciplinary & Community Coordination
Communicate regularly with psychiatric providers, primary care, hospitals, and specialists
Coordinate durable medical equipment and community-based care supports
Provide medication-related training and consultation to internal teams
Ensure notes meet agency, OAR, and Medicaid billing standards
You are licensed as an LPN in the State of Oregon
You have at least2 years of experience in professional nursing (mental health or residential preferred)
