Summary of Duties

As a Human Services Case Manager, you will:

Determine initial and on-going Medicaid eligibility for long term services and supports (LTSS), to develop and implement service plans, to coordinate the provision of services for community-based care and nursing homes, and to regularly review and update Medicaid eligibility and monitor service plans.

Major duties include:

  • Screen and interview applicants/recipients, relatives, and other interested persons in the field, to determine initial and on-going eligibility for Long Term Care Services, health care, social service, diversion/transition, and protective service's needs. Inform clients of rights and methods of recourse.
  • Develop detailed, individualized case management plans for clients. Conduct on site field reviews using OR ACCESS, CA/PS assessment tool and the laptop computer. Provide on-going case management by coordinating the provision of services to clients according to rules, regulations, and the individual case plans. Conduct (direct and indirect) contacts with consumers or their authorized representatives.

Minimum Qualifications

A valid driver's license and acceptable driving record are required for this position.

  • A bachelor's degree in a Behavioral Science, Social Science, or a closely related field.
  • OR A bachelor's degree in any field AND ONE YEAR of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
  • OR An associate degree in a Behavioral Science, Social Science or a closely related field AND TWO YEARS of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
  • OR Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

  • Experience effectively working independently, using problem solving and critical thinking skills
  • Experience efficiently managing a workload in a constantly changing work situation, as well as plan for and resolve complex problems.
  • Experience analyzing information or situations to make good decisions within program rules and policies
  • Demonstrated ability to coordinate and maintain consistent communication with consumers, colleagues, and community partners via phone, email, in-person meetings, and virtual platforms such as Microsoft Teams to ensure timely follow-up and service coordination.

Working Conditions

  • This position requires work on site and is not a remote position.
  • The office is open from 8am-5pm, Monday through Friday. The needs of the public will be adequately served.
  • The work schedule for this position is a 40-hour work week. May require some overtime work before and after scheduled work hours and on weekends.
  • General office conditions. This position also includes an office environment that includes long periods of sitting for data entry, document reviews and desk side interviewing.
  • Daily ongoing use of a keyboard and CRT screen are necessary. There may be physical exertion such as climbing stairs, walking on uneven terrain and transporting a laptop.
  • May come into contact with irrational, hostile or angry people who could become physically aggressive or have weapons.
  • May come into contact with persons with communicable diseases such as staph infections, Hepatitis, MRSA or lice.
  • This position requires a valid driver's license and an acceptable driving record. If not, you must be able to provide an alternate method of transportation.
  • This position requires travel of more than 100 miles per one-way trip. You must travel to homes of clients, hospitals, nursing facilities and other facilities as well as other field offices and to Salem.
  • Travel may be in inclement weather.
  • This position requires a good attendance pattern, and appropriate use of accrued paid leave in order to ensure branch coverage for vacations and other leaves, as well as acceptable customer service for our clients, community partners and the general public.

  • The Oregon Department of Human Services does not offer visa sponsorship.
