Job Description
Job Title: Licensed Practical Nurse (LPN)

Profession: LPN/LVN

Duration: 13 weeks

Shift: 12H Nights, every other week required

Hours per Shift: 36 hours per week

Experience: Minimum 2 years of acute care experience

License: Current Oregon State LPN licensure

Certifications: BLS Certification

Must-Have: Proficiency in basic nursing skills, patient assessment, and documentation

Description:
The Licensed Practical Nurse (LPN) in the Discharge Unit works under the supervision of the RN to provide direct patient care for patients medically ready for discharge.

This role involves working collaboratively with a multidisciplinary team.

The LPN assigns and supervises tasks to other LPNs, Certified Nursing Assistants (CNAs), and unregulated assistive persons (UAPs).

The LPN maintains an in-depth knowledge of nursing principles, practices, standards, and techniques, applying knowledge in accordance with established policies and procedures.

The LPN must have strong communication and interpersonal skills.

The LPN must be flexible and adaptable in a dynamic healthcare environment focused on discharge readiness.

The LPN's practice includes the provision of direct patient care, performing assessments, and implementing and evaluating care plans as directed by the RN.

The LPN communicates patient status and any concerns regarding the RN's plan of care.

The LPN may assign focused plan of care interventions to other staff according to their qualifications and patient needs.

The LPN is responsible for documenting assessments, interventions, patient responses, and progress in the electronic health record.

The LPN assists in providing patient and family education, reinforcing discharge plans, medication instructions, and follow-up care needs.

Effective communication with the interdisciplinary team is essential to ensure continuity of care, timely updates, and smooth transitions.

Adhering to all policies, procedures, and protocols is required.

The LPN must report any concerns promptly to the charge nurse or unit manager.

Participation in unit-based quality improvement initiatives to enhance patient outcomes is encouraged.

The LPN must demonstrate awareness of cost containment and the economical use of supplies and equipment.
