Job Description
The Care Manager is responsible for providing comprehensive care management services to beneficiaries with planned admissions to acute care hospitals, or those transitioning from case or disease management programs.The role aims to systematically identify and address fragmented care for patients with acute, real-time needs, tailoring interventions to fill gaps and achieve optimal clinical outcomes cost-effectively.
Responsibilities

  • Conduct pre-admission counseling by contacting patients with upcoming hospital admissions to discuss expectations.
  • Assess patients' conditions to understand their illness or injury and evaluate their ability to follow the treatment plan.
  • Advise patients on the probable length of stay and help arrange for services post-discharge.
  • Collaborate with physicians and hospitals to enforce treatment plans and ensure the patient receives necessary specialty care and tests.
  • Coordinate health care team services, avoiding duplication and conserving benefit dollars.
  • Evaluate the need for and authorize equipment, supplies, and services.
  • Instruct patients and families in proper care and refer them to appropriate health care team members as needed.
  • Conduct hospital visits and confer with physicians to clarify diagnosis, prognosis, and therapies.
  • Document case summaries in the Transitional Care Plan and share appropriately with beneficiaries and providers.
  • Facilitate beneficiary transfers among regions and collaborate with military liaisons to minimize disruption of care.
  • Assess patients' benefit plan coverage and negotiate cost-effective rates for provider services.
  • Suggest medically appropriate alternatives to achieve treatment plan goals cost-effectively.
  • Contact patients within 48 hours of discharge to ensure sufficient support for full recovery and assess compliance with medications and follow-up appointments.
Essential Skills
  • Graduate of a Nursing program, BSN Degree preferred, or a degree in Clinical Psychology or Clinical Social Work.
  • Must have and maintain a current, valid, and unrestricted Registered Nurse license.
  • Minimum of 2 years of experience in a relevant field.
  • Proficient in case management, managed care, and acute care.
  • Proficient in Microsoft Office applications including Excel, Outlook, and Word.
Additional Skills & Qualifications
  • Spanish speaking is preferred for bilingual communication.
  • Experience in home health, hospice, or hospital settings is advantageous.
  • Strong computer skills are required.
Work Environment
This is a remote work position requiring strong Wi-Fi and a dedicated workspace with a closed door. The work schedule is Monday through Friday, from 9:00 AM to 6:30 PM EST initially, with the possibility of transitioning to a four-day workweek after demonstrating proficiency. Candidates must be open to this flexibility.
Job Type & Location
This is a Contract to Hire position based out of Atlanta, GA.
Pay and Benefits
The pay range for this position is $35.00 - $41.00/hr.
