Provides client centered care utilizing evidence-based and trauma informed services. Conducts assessment interviews using evidence based tools and provides therapeutic interventions to address identified treatment and recovery goals. Manages an appropriate sized case load of clients. Implements treatment planning goals, utilizing program-specific evidence-based practices. Works collaboratively with treatment team to meet client goals. Develops and facilitates client groups. Coordinates care with medical providers, mental health providers, and other service providers. Provides timely accurate documentation of services in electronic health record.
Able to satisfactorily perform the requirements of the job. CADC certification or willingness to work towards certification. Computer proficiency required to use Electronic Health Records. Valid OR driver's license and excellent driving record required. Must successfully pass state-required background and DMV checks.
